Therapy in a Patient Driven Groupings Model

Kornetti & Krafft Health Care Solutions provides an interactive conference that addresses the paradigm shift from a volume- to a value-based care for therapists in the home health setting. This one-day course will provide training and resources related to therapy in a patient-driven acuity model of reimbursement with special attention paid to specific chronic disease populations commonly encountered in this setting. Focus will be to highlight PDGM with evidence-based practice recommendations provided to support the role of restorative and maintenance therapy in risk and outcome management.

Course attendees will receive current regulatory guidance and expert content to support clinical care for patients in each of the designated Patient-Driven Groupings Model categories. Inclusion of current OASIS data set information will be discussed, as well as tools and resources supporting clinical care coordination, decisioning and defensible documentation.

Objectives:

  1. Articulate the components of PDGM and how they are impacted by clinical decision making.
  2. Develop specific strategies to address patient issues in commonly treated diseases, diagnoses and conditions seen in the home health setting within the context of a value- and outcome-driven model.
  3. Ensure documentation throughout the record shows intentionality with respect to patient centered care planning and attention to outcome measures.
