Advanced Care Planning - What’s the Right Thing to Do?
Ellie Ward BSN, RN, Community Education and Outreach- Advance Care Planning, Vidant Health

Working in healthcare and as leaders, we are faced with challenging ethical situations.  Determining the “right” course of action can be challenging and fraught. This is often the case in many end-of-life care situations.  What are the patient’s wishes? What are their goals of care, or desired outcomes? Have they told anyone? Are their decision makers adequately prepared to represent them?  Have we asked the right questions in order to advocate for our patients’ preferences and priorities?  Join us for an interactive case study and discussion around advance care planning and steps we can all take to better understand what our patients want, and how we can collectively serve as change agents in order to honor patient’s wishes within our industry and across all care settings.
Take the Lead by Example challenge by initiating difficult, but outcome changing conversations with your family and loved ones, and by completing your own personal advance directives!

Memories formed at a young age are the last to die off, which is why long-term memory is prevalent in those diagnosed with Dementia. The use of music specifically targeted to the era of the patient and personalized music selections that had meaning to the patient can enhance the temperament and behavior of the client. It can suppress depression, raise awareness, and bring back words and speech which had previously disappeared. It showcases meaningful quiet time for the client and the caregiver and has been proven to change a person’s perspective in the days ahead.

In this session, this topic will be discussed through the eyes of a former PCA, turned Tech CEO. The hard costs are clear, but the soft costs can be harder to identify and even harder to address. Communicating Diagnosis Conventions and Other Rules to Hospice Medical Directors and Others That Need to Know
Lisa Selman-Holman, JD, BSN, RN, HCS-D, HCS-O, COS-C, Selman-Holman, A Briggs Healthcare Company

For years hospices were able to code “one and done” and did not worry about coding guidelines. Over the past few years Medicare has made it clear that hospices are not only to follow coding guidelines, but to code all the diagnoses, not just the terminal diagnosis. In getting the diagnoses information needed, we sometimes run into roadblocks. Compliance with coding guidelines while complying with Medicare requirements requires communication in the IDT and often education of the staff and medical director.

Affecting Positive and Lasting Behavioral Change in Your Post-Acute Organizations - a Change Management Presentation
Jennifer Maxwell, M ed, President and co-Founder,  Maxwell Healthcare Associates

With regulations, technology and research consistently evolving, how do you best manage change in your organization to ensure it’s worthwhile, relevant and long lasting? Organizations that can adapt and evolve better and faster than the competition... win. While many healthcare providers have established processes for rolling out changes and training team members accordingly, often the process falls short because the team members’ behaviors were superficially adjusted. Lasting and positive change occurs when human behavior is tranformed at its core. In this session, popular psychological behavioral change and change management theories will be discussed as well as barriers to change and ways to communicate change.

CMS has a number of tools it can utilize when it finds what it believes to be fraudulent or abusive behavior. Managed Care Plans are also now running rampant with requests for documentation. Payment suspensions and PTAN revocations are two different tools that are being utilized more frequently than in the past. These audits also result in referrals to the Office of Inspector General for further evaluation. Knowing what to expect and knowing what tools are available to CMS can make a difficult situation less challenging. This session will discuss what things can cause an audit to go wrong and what steps suppliers can take to minimize the damage.
